Anemia is the most common nutritional deficiency in the world, and children are among the most vulnerable. It happens when the body doesn’t have enough healthy red blood cells to carry adequate oxygen to tissues and organs, and the effects go beyond fatigue. Left unaddressed, anemia in growing children can affect development, cognition, and long-term health in ways that aren’t always obvious in the early stages. What Causes Anemia in Children
Iron deficiency is the most common cause of anemia in children by a significant margin, but it’s not the only one. Understanding what’s driving a child’s low red blood cell count shapes how we approach treatment.
Iron deficiency anemia develops when the body doesn’t have enough iron to produce hemoglobin, the protein inside red blood cells that carries oxygen. In infants and toddlers, this often traces back to diet: too much cow’s milk crowding out iron-rich foods, insufficient iron-fortified foods after weaning, or breastfeeding without iron supplementation. In older children and teens, poor dietary habits, rapid growth spurts, and for adolescent girls, menstrual blood loss are the primary drivers. A 2023 analysis found that nearly 39% of females between ages 12 and 21 in the U.S. are iron-deficient, making teen girls one of the highest-risk groups.
Other causes of anemia in children include deficiencies in vitamin B12 or folate, which affect how red blood cells are produced. Genetic conditions such as sickle cell disease and thalassemia cause the body to break down red blood cells faster than it can replace them. Chronic illness, certain infections, and some medications can also suppress red blood cell production. In less common cases, the bone marrow itself fails to produce enough cells.
Signs of Anemia That Parents May Overlook
Many children with mild anemia have no obvious symptoms, which is exactly why it often goes undetected until a routine blood test flags it. When symptoms do appear, they tend to be gradual and easy to attribute to other causes.
Fatigue and low energy are the most common signs, but in children these can look like general moodiness, reduced interest in activities, or trouble concentrating at school. Pale skin, particularly noticeable in the face, lips, and inner eyelids, is another tell. A blanched inner eyelid (the tissue lining the inside of the lower lid) correlates with anemia in a significant percentage of pediatric cases and is something a provider will check during an exam.
Less commonly recognized signs include rapid or irregular heartbeat, shortness of breath during normal activity, frequent headaches, and dizziness. In children with iron deficiency specifically, pica is a notable symptom: an unusual craving for non-food substances like dirt, clay, or ice. It sounds unusual, but it’s a recognized clinical marker and worth mentioning to a provider if you’ve noticed it.
In infants, signs of anemia can include poor feeding, slow weight gain, and delayed developmental milestones. Because many of these overlap with other common childhood concerns, blood testing is the only reliable way to know what’s actually going on.
How Anemia Is Diagnosed and Treated
Diagnosis starts with a complete blood count (CBC), a blood test that measures red blood cell levels, hemoglobin, and other markers that help identify the type and severity of anemia. The AAP recommends universal hemoglobin screening at 12 months for all infants, with additional screening at 9 to 12 months for high-risk children. For adolescent girls, screening after the onset of menstruation is also recommended given the elevated risk from monthly blood loss.
If the CBC points to iron deficiency, we may also check ferritin levels, which measure the body’s stored iron. A child can be iron-deficient before their hemoglobin drops low enough to be classified as anemic, so catching falling ferritin early gives us more room to act before the deficiency deepens.
Treatment depends on the cause. For nutritional anemia, dietary changes are the foundation. Iron-rich foods including red meat, poultry, beans, fortified cereals, and dark leafy greens are prioritized. Pairing iron-rich plant foods with vitamin C improves absorption meaningfully. Reducing excess cow’s milk in toddlers (more than 24 ounces per day can interfere with iron absorption) is often part of the plan as well.
Iron supplementation is typically recommended when dietary changes alone aren’t sufficient or when deficiency is confirmed and needs to be corrected more quickly. Dosing is based on the child’s weight and the severity of deficiency. Follow-up blood work is done after a period of treatment to confirm levels are improving. For anemia with a genetic or complex medical cause, management is more involved and may require specialist referral.
When Professional Guidance May Be Helpful
If your child seems persistently tired, looks pale, is a picky eater with a diet low in iron-rich foods, or has recently entered adolescence especially if they’ve started menstruating, those are all good reasons to have their blood levels checked. The same applies if your infant or toddler is drinking a lot of cow’s milk or wasn’t supplemented with iron during breastfeeding.
Catching anemia early, or catching iron deficiency before it becomes anemia, makes treatment more straightforward and protects your child’s development during the years when it matters most.

Can anemia affect my child's performance at school?
Yes, and this is one of the more consequential effects in school-age children. Iron deficiency, even before it progresses to full anemia, has been linked to attention difficulties, slower information processing, and reduced academic performance. The brain depends heavily on iron for dopamine regulation and neurological function. Children who are iron-deficient often show measurable improvements in focus and cognitive performance once levels are corrected.
Is it possible for a child to have normal hemoglobin but still be iron-deficient?
Yes. Iron deficiency develops in stages. Ferritin (stored iron) drops first, often well before hemoglobin falls low enough to register as anemia on a standard blood test. A child can feel the effects of low iron stores, including fatigue and difficulty concentrating, while still appearing “normal” on a basic hemoglobin check. That’s why we look at the full picture, including ferritin, when there’s a clinical reason to investigate.
Does cooking in a cast iron pan really add iron to food?
It does, in small amounts. Acidic foods cooked in cast iron, such as tomato-based dishes, absorb the most. It’s not a substitute for dietary iron sources or supplementation when deficiency is present, but for families managing borderline iron intake, it’s a legitimate and easy add.
How does cow's milk contribute to anemia in toddlers?
Cow’s milk interferes with iron absorption and, when consumed in large amounts, can displace iron-rich foods in a toddler’s diet. It also contains very little iron itself. Toddlers who drink more than 24 ounces of cow’s milk per day are at significantly higher risk for iron deficiency anemia. Whole milk is appropriate in moderate amounts after age one, but it shouldn’t be the centerpiece of a toddler’s diet.
If my child is a vegetarian or vegan, how concerned should I be about anemia?
Plant-based diets can meet iron needs, but it requires more intentional planning. Plant-based iron (non-heme iron) is absorbed less efficiently than iron from animal sources. Pairing plant-based iron foods like lentils, tofu, fortified cereals, and spinach with vitamin C at the same meal improves absorption significantly. Children on fully plant-based diets benefit from regular blood monitoring to catch any developing deficiency early.
